If, in the judgment of any representative of Prosper ISD (PISD), my registered student should need immediate care and treatment as a result of any injury or illness, I grant authorization and consent for such care and treatment to be provided to the student by any physician, athletic trainer, nurse, hospital, or school representative.
In the event of serious injury or illness, an attempt will be made by a PISD representative, attending physician, or nurse to contact the parent or legal guardian in the most expedient manner possible. If direct communication is not possible, I grant authorization for the treatment necessary for the welfare of the registered student.
I provide this authorization in advance of any specific diagnosis, treatment, or hospital care being required and my authorization is intended to provide authority and power on the part of the aforesaid agent(s) to give specific consent to any and all such diagnosis, treatment, or hospital care deemed advisable.
Prosper ISD is authorized to photograph, record, or otherwise capture the image, likeness, or voice of the registered student during summer camp activities. These images or recordings may be used for educational, informational, or promotional purposes related to Prosper ISD programs, including but not limited to district websites, social media platforms, publications, presentations, and marketing materials. No compensation will be provided for the use of these materials, and all rights to such media remain the property of Prosper ISD.
A parent or legal guardian who does not wish for their student to be included in photographs or recordings may opt out by notifying the camp sponsor in writing via email no later than 24 hours prior to the start of the camp. Opt-out requests received after this timeframe may not be able to be accommodated.
Registration for a Prosper ISD summer camp constitutes acknowledgement and acceptance of the notices and authorizations outlined above.
